FORT POLK OFFICERS' SPOUSES' CLUB WELFARE COMMITTEE GUIDELINES The goal of the Fort Polk OSC Welfare committee is to promote community interests, support worthy causes and improve the quality of life for Fort Polk and our surrounding communities. Requests for donations will be researched and reviewed twice a year. The Fort Polk military community will be our priority when considering disbursement of funds. During the review, we will evaluate the impact the request has on the community as well as the number of military families affected in regard to education, health, welfare, food and shelter. Following are some basic guidelines that will be used when evaluating requests for funds: -All applicants must complete the appropriate application forms and submit them no later than the deadline dates set forth by the Welfare committee. Exceptions to the deadlines will be considered and reviewed by the Welfare committee on a case-by-case basis. -For educational institutions we will attempt to benefit the entire student population. Donations cannot exceed $500 per operational year. -Private non-profit organizations must provide proof of 501C status. -For an emergency situation (i.e. house fire, death) affecting any military family stationed at Fort Polk, the donation cannot exceed $250. -Requests from organizations that benefit the general population (i.e. Literacy councils, libraries, local museums, etc.) will also be considered. -Non-appropriate funds organizations are not eligible to request funds. (In accordance with AR 210-1) However, the committee reserves the right to donate funds for activities that benefit Fort Polk families. -Other ineligible groups include, but are not limited to: private individuals, Family Readiness Groups, military units, and non-profit organizations. -Disbursements for child care will be limited to a maximum of 10 children (8 hours of child care provided per child) for approved request. The hourly fee will be based on the current CDC rates and the total will be paid directly to the CDC.
The Welfare committee will try to provide as much funding as possible, however, the amounts of funds awarded will be in direct relation to the success of the OSC fundraisers for this operational year.
